Medicar works with care homes and nursing homes to arrange suitable adult patient transport with clear collection, journey and handover plans.
A resident may need transport for a hospital appointment, discharge, a move to a new placement, respite care, a family occasion or another planned journey. The care team often holds the information needed to assess the journey accurately.
Medicar can coordinate with the sending home, receiving service, hospital or family. We ask about the resident's transport position, cognition, mobility, comfort, access and any escort requirement before confirming the booking.
For new placements and hospital discharge, both locations should agree the expected time and who will complete the handover. Medication, paperwork and belongings remain the responsibility of the relevant care or clinical team unless specifically agreed otherwise.
Provide a named person and direct number for collection and destination. If the journey timing changes, Medicar needs a reliable route to confirm the update.
What a care home should provide
A short but complete handover of journey information allows a realistic transport plan.
- Resident's required transport position
- Relevant mobility and transfer support
- Collection and destination access
- Appointment or handover time
- Whether a staff escort will travel
- Named contacts and direct telephone numbers
Supporting planned resident journeys
Medicar's service is for adults aged 18 and over and is planned rather than emergency response. A change in the resident's condition should be reviewed by the appropriate clinical professional and shared before travel.
Care providers can request quotations for individual or repeat journeys. Any referral or commission arrangement would need separate written terms and is not created by making a booking enquiry.
Frequently asked questions
Can a care home book directly with Medicar?
Yes. A care home, nursing home or authorised representative can request a private quotation and provide the resident's journey information.
Can a member of care staff travel with the resident?
This may be possible depending on the assessed vehicle, passenger needs and available seating. Include the escort requirement in the enquiry.
Can you collect a resident after hospital discharge?
Yes, suitable planned discharge transport can be arranged when the ward confirms readiness and the care setting is ready to receive the resident.
Can we arrange regular transport?
Repeat journeys can be discussed and scheduled subject to suitability, availability and agreed booking terms.
